NASAA sets the Series 66 retake waiting periods:

After a 1st fail: minimum 30 days
After a 2nd fail: minimum 30 days
After a 3rd fail and every attempt after: minimum 180 days

There is no lifetime limit on attempts. Each retake requires a new enrollment (through your firm's Form U4 or self-enrollment) and a new $177 fee, and each enrollment opens a fresh 120-day scheduling window.
